Consent Form for Collection, Use, and/or Disclosure of Personal Data: Initial Public Observation on Geographical Indication Protection under the Thailand-European Union Free Trade Agreement (FTA) Negotiations

เนื่องจากการรับฟังความเห็นเบื้องต้นเกี่ยวกับสิ่งบ่งชี้ทางภูมิศาสตร์ของสหภาพยุโรปภายใต้การเจรจาความตกลงการค้าเสรี (FTA) ระหว่าง ไทย-สหภาพยุโรป (EU) จำเป็นต้องใช้ข้อมูลส่วนบุคคลของท่านในการประมวลผลข้อมูลส่วนบุคคล กรมทรัพย์สินทางปัญญาจึงขอความยินยอมจากท่านในการเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคล โดยจะเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลเท่าที่จำเป็นเพื่อวัตถุประสงค์ ดังต่อไปนี้

  1. เพื่อใช้ตรวจสอบ และยืนยันตัวตนของท่านในการให้ความเห็นเบื้องต้นเกี่ยวกับสิ่งบ่งชี้ทางภูมิศาสตร์ของสหภาพยุโรป
  2. เพื่อวิเคราะห์และประมวลผลความเห็น โดยกรมทรัพย์สินทางปัญญาจะใช้ข้อมูลส่วนบุคคลของท่านสำหรับการวิเคราะห์ทางสถิติ
  3. เพื่อการติดต่อประสานงานเพิ่มเติม หากมีข้อสงสัยหรือต้องการข้อมูลเพิ่มเติม

Your personal data is required for processing due to the Initial Public Observation on Geographical Indication Protection under the Thailand-European Union Free Trade Agreement (FTA) Negotiations. Therefore, the Department of Intellectual Property (DIP) requests your consent to collect, use, or disclose your personal data. This data will be collected, used, or disclosed only as necessary for the following purposes:

  1. To verify and confirm your identity for providing initial feedback on the geographical indications of the European Union under the Thailand-European Union FTA negotiations.
  2. To analyze and process feedback, the DIP will use your personal data for statistical analysis to better understand your opinions.
  3. To facilitate further contact and coordination if there are any questions or if additional information is needed beyond what you have already provided.

The DIP will safeguard your personal data and implement appropriate security measures in accordance with Cybersecurity guidelines and the standards stipulated by the Personal Data Protection Act B.E. 2562 (2019) and other relevant laws.
