| 1 | The trademark owner and permittee/sub permittee’ s national ID card or other ID card issued by a governmental agency (1 copy) in case of an ordinary person. |
| 2 | The trademark owner and permittee/sub permittee’ s Certificate of Alien Registration or passport (1 copy) in case of a foreigner, except the applicant submits the authorization letter or power of attorney made outside the Kingdom of Thailand as stated in item 4. |
| 3 | The trademark owner and permittee/sub permittee’ s Certificate of Juristic Person issued not more than 6 months prior to the application submission date ( 1 original document), in case the applicant is a juristic person. If the applicant is a juristic person established under the foreign law, and submits the documents according to item 4 with Certification of Juristic Person established abroad, the applicant is not required to submit the Certificate of Juristic Person. |
| 4 | Authorization letter or power of attorney and ID card of representative or agent/attorney (1 copy) with the revenue stamp of 30 Baht per representative or true agent/attorney. |
| 5 | Application for registration of trademark use license agreement (Kor 05 Form) (1 original document) |
| 6 | The license agreement signed by the trademark owner and permittee (1 original document/ 1 copy). The original contract shall be presented to the official for inspection. Then, the official shall return the original contract to the applicant and keep only its copy. |
| 7 | The sub license agreement signed by the permittee and sub permittee (1 original document/ 1 copy) 1. In case of sub-license 2. The original sub-license agreement shall be presented to the official for inspection. Then, the official shall return the original contract to the applicant and keep only its copy. |
| 8 | The registration certificate with explanation why cannot be presented (1 original document) |
